Medicare Covers Many Vaccines for Free

As we age, protecting our health is more important than ever. We know older adults tend to experience severe symptoms and complications from many communicable viruses and diseases, and can have a harder time bouncing back from them. Being vaccinated lowers the risk for serious illness, and in extreme cases death; and the side effects of vaccines are typically mild and temporary. While Medicare covers the cost of vaccines for the flu, COVID-19 and RSV, many seniors are not always sure if or when they need to get another shot. 

The Center for Disease Control and Prevention (CDC) recommends people 65 years or older get an annual high-dose flu vaccine if available as they suffer the most from the disease. There are two types. One high-dose flu vaccine contains an antigen called hemagglutinin and has 3 to 4 times as much flu virus antigen as a standard vaccine. The other has an ingredient called an adjuvant that helps the immune system respond better to the virus. If you are over 65 and suspect you have the flu, talk to your healthcare provider as soon as possible as they can prescribe a flu antiviral treatment. 

The current recommendation for the COVID-19 vaccine is for people 65 years or older to get 2 doses of the updated 2024-2025 vaccine six months apart. They say if you have upcoming travel or events in your life where you want to be very protected from the disease you could get your second dose somewhat earlier (minimum interval of two months). 

The respiratory syncytial virus (RSV) vaccine protects against the respiratory syncytial virus, and the CDC recommends all persons 75 years or older get the one time shot. If you are someone at an increased risk, they say people ages 60-74 years old can get the RSV vaccine. 

Other vaccines covered by Medicare Part D include vaccines for shingles, tetanus-diphtheria-whooping cough (TDAP) and others. Consult with your healthcare providers on what vaccines you should be getting to protect your health.

If you are an older adult who wants to start exercising again, you may not know how to begin.  For one thing, you probably realize you are not as fit as you used to be, and are not up to the same degree of physical activity that you once did.  And of course, before you begin any new form of exercise, you will need to check with your doctor to make sure you are fit enough, especially if you have osteoporosis, an injury, or balance issues.  

Once you get the go ahead from your healthcare provider, stretching is a good place to begin.  In fact, studies show that as we grow older our flexibility decreases by up to 50 percent in some joints.  Another good thing about stretching is many exercises are appropriate for any level of fitness including ones that can be done sitting in a chair.  Stretching can help you maintain your flexibility and mobility as well as loosen up some of your stiffness in your back, neck and shoulder muscles.

Another plus about stretching exercises is that most of them do not require special equipment and can be done anywhere.  The Silver Sneakers website recommends that as you stretch, “to breathe deeply, and go slowly. Listen to your body, and never force a movement that causes pain.”  They remind seniors it is okay if they cannot bend very far as over time they will loosen up some.  Safety is always important and you do not want to hurt yourself.  Other exercises especially good for seniors include chair yoga, resistance band workouts, walking, and dumbbell strength training.

Help Delay the Onset of Diabetes With Lifestyle Changes

Type 1 and Type 2 diabetes affects millions of Americans, and every year more than a million people are diagnosed with the disease. The American Diabetes Association (ADA) reports in 2021, 38.4 million Americans, or 11.6% of the population, had diabetes. Of that number 2 million had type 1 diabetes, which includes more than 300,000 children and adolescents. 

Seniors aged 65 years and older make up a large percentage of Americans with the disease at 29.2% or 16.5 million seniors (diagnosed and undiagnosed). Diabetes can be deadly and was the 8th leading cause of death in 2021. People with diabetes are at an increased risk of vision problems, heart attacks, nerve damage and stroke. In 2022, the CDC estimated the cost of diabetes was $412.9 billion. 

Prediabetes is another huge problem and numbers from 2021 report 97.6 million Americans aged 18 and older are affected. It is a chronic condition in which blood glucose levels are higher than normal but not high enough to be classified as diabetes. While it carries an increased risk of developing type 2 diabetes, it isn’t inevitable. Studies show prediabetes can be reversed or the risk mitigated with lifestyle changes such as diet, medications, and early detection. 

Yale Medicine endocrinologist, Anika Anam, MD, explains, “Prediabetes goes hand in hand with metabolic syndrome, the term for a condition that includes hypertension, obesity, and high cholesterol.” She adds each of those conditions raises the risk for serious issues including heart disease, stroke, and cancer. 

The CDC’s national Diabetes Prevention Program (DPP), finds “people with prediabetes who lost a modest amount of weight—5 to 7% (10 to 14 pounds for a person weighing 200 pounds, for example)—with the help of a structured lifestyle-change program cut their risk of developing type 2 diabetes by 58%.” In addition, experts say at least 30 minutes of moderate exercise per day can not only improve insulin sensitivity and lower blood sugar levels, but aid in weight loss. Cutting out added sugars, eating more complex carbs (vegetables, legumes, nuts, and whole grains), and increasing protein are other recommended lifestyle changes as is reducing stress.

Age-Related Macular Degeneration Affects Nearly 20 Million Americans

Just as the rest of our body ages, so do our eyes. Older adults often complain of dry, teary or tired eyes and may have problems driving at night, experience flashes and floaters or need glasses to read. As we grow older, we are also at a higher risk for serious eye diseases that can impact our vision. 

One of the most serious eye diseases is age-related macular degeneration (AMD). The American Macular Degeneration Foundation reports AMD is the leading cause of irreversible vision loss among people aged 60 and older, affecting nearly 20 million Americans. The risk of developing AMD increases with age and if you are 75 or older, your risk is 1 in 3.  Age-related macular degeneration causes the macula, a part of the back of the eye near the retina, to deteriorate. Experts say while it doesn’t impact peripheral vision, macular degeneration can affect how well a person sees what’s right in front of them.

You are at a higher risk of getting AMD if a relative has had it or you are Caucasian, and females are more susceptible to AMD than males. Lifestyle choices also increase your risk for AMD including smoking along with elevated blood pressure and cholesterol levels. Sun exposure is another risk factor and doctors recommend wearing sunglasses and a wide-brimmed hat. Maintaining a healthy diet by eating foods high in antioxidants like lutein found in green, leafy vegetables such as spinach are beneficial for our eye health.

Symptoms of AMD include visual distortions and can make straight lines appear bent, reduce central vision in one or both eyes, increase blurriness of printed words, and the need for brighter light when reading or doing close-up work. The first signs of AMD are typically discovered by an eye doctor during an annual dilated eye exam.

The NIH also explains those who felt better about their experience practiced communication skills, were organized, talked to peer mentors and kept a positive attitude. 

Research shows practicing self-advocacy, or when a family member actively advocates on the behalf of a loved one, reduces medical errors in hospital settings, doctors’ offices or at the pharmacy. It is important as the World Health Organization reports “an estimated 10% of patients in high-income countries are harmed while receiving hospital care.” Also, according to a 2021 report in StatPearls, “medical errors result in about 100,000 deaths in hospitals and clinics annually.”  

Most errors are related to diagnoses, prescriptions and use of medicines; and misdiagnoses means delayed treatment and taking the wrong medicines. To ensure the best care you need to be honest with your healthcare provider. This includes providing an account of your symptoms and a list of current prescriptions, over-the-counter medications, herbs, supplements and vitamins you take. Be sure to let them know if you have any drug allergies and provide as complete a medical history as you can including your personal medical record as well as that of your family’s. Also, let your medical team know if you are a smoker and the amount of alcohol you consume. 

Additionally, remember the time you spend with your doctor is short, so prepare beforehand the things you wish to discuss. The Johns Hopkins Armstrong Institute Center for Diagnostic Excellence at Johns Hopkins Medicine in Baltimore has a QuestionBuilder app available by AHRQ at Google Play or the Apple store to guide you in questions you should ask. 

Experts also say to remember you and your doctor are a team. Being your own health advocate or that of a loved one means helping them follow the medical advice recommended by the doctor regarding diet, rest, exercise, smoking, alcohol use and taking medications as prescribed.

Helping Children Understand Alzheimer’s Disease

When a grandparent or another close family member or friend suffers from Alzheimer’s or another form of dementia, it can be difficult for children to understand, especially if they are very close to the person.  Most likely they have questions and concerns regarding the disease and may be struggling with a range of emotions including sadness and confusion. They may not know how to act around the person anymore or be frightened by the “strange behavior.”

Having grandma or grandpa call you by mom or dad’s name or asking the same question again and again are just some of the strange things kids see happening to a beloved grandparent. They may worry Alzheimer’s is contagious and it is important they know it isn’t but rather a degenerative disease that happens in the brain. If the person lives with them, teenagers may be embarrassed to have friends around due to the person’s behavior, and sometimes children withdraw from the person or lose patience.  

The Alzheimer’s Association suggests sitting down with your child and telling them while the person may forget your name, they can still feel your love. Encourage questions and provide honest answers, and perhaps together you can learn more about the disease. The experts offer ideas on things kids can do with the person such as listening to music, setting the table, working on a puzzle, reading a book or looking at family photos. As the person’s disease progresses, make sure your kids feel comfortable asking more questions. Regardless of how old we are, losing a loved one to Alzheimer’s disease is challenging for everyone.
