Let’s be honest, many of us rely on the Internet for medical advice. While it can supply good answers, doctors also caution you have to be careful what sites you trust. There are many websites providing good medical information but there are also sites with misleading or even dangerous advice.

Dr. Michael Langan, an internal medicine physician at Ohio State University Wexner Medical Center, cautions anyone with a blog can dispense information and advice; and a good way to avoid misinformation is to ask your doctor to recommend the sites they trust. Additionally, the National Institute on Aging recommends using health websites sponsored by Federal Government agencies. They say a good place for reliable, up-to-date information is the National Institutes of Health, but at the same time, remind everyone that the Internet is no substitute for seeing your doctor or other health professional who can give you advice that caters to your specific situation. 

Other things to look for include name recognition such as the Mayo Clinic or the Cleveland Clinic. Moreover, remember sites that end in .gov, identity U.S. government agencies; .edu, educational institutions; and .org, usually designates nonprofit organizations (such as professional groups; scientific, medical, or research societies; and advocacy groups). A .com is a commercial site.

The NIH says to look for the date of the information (often can be found at the bottom of the page) and see if the authors and contributors are identified. If the author is listed, what is his/her background and what is their goal in writing the article or the intent of the organization it is written for. Is there any financial motive for the information? Check out the About Us or Contact Us pages as they provide background information about the organization.

As we age, many seniors experience occasional “brain fog” or forgetfulness and have trouble comprehending and processing new information. Medical experts say this may be a normal part of aging but could also be from some of the medications you are taking. 

Dr. Mark Albers, a neurologist at the McCance Center for Brain Health at Harvard - affiliated with the Massachusetts General Hospital, says it is not all that uncommon for seniors to experience symptoms of brain fog when taking certain medications for the first time. He explains medications can affect memory more in aging adults as “older people tend to metabolize drugs more slowly, making them more sensitive to medications and more vulnerable to side effects.” 

Other reasons medications cause more problems in seniors are they often take larger doses of a drug and more frequently as well as take several drugs to treat one health condition. Moreover, older people may have a weakened blood-brain barrier. This barrier works to block toxins and other harmful substances including drugs from entering the brain, which may account for some memory problems. 

Sleep medications are one of the common medications that can be problematic. Over-the-counter sleeping aids can contain anticholinergic drugs, which are known to impair cognitive function, and some prescription sleep drugs diminish activity in parts of the brain involved in transferring short-term memories to long-term memory. Pain medications can additionally result in memory problems. The good news is once patients stop taking sleep or pain meds, modify the dose or replace the drug with a better option, many of the problems go away.

It’s Never Too Late to Improve Your Health and Mobility

Even if you have already moved into the “senior” category of life, medical experts say it is not too late to improve your health and mobility, and one of the best ways to do it is to walk. These were the findings based on the Lifestyle Interventions and Independence for Elders (LIFE) trial, which included more than 1,600 men and women between the ages of 70 and 89. Participants consisted of people who were not regularly exercising and fairly frail.  

The participants were divided in half with one group assigned to daily walking and strength and balance exercises. The other group attended educational workshops and were introduced to gentle stretching routines. “After 2½ years, the volunteers in the exercise group were 28% less likely to have become disabled (defined by the inability to walk about 400 yards without help) compared to those in the education group. They were also 18% less likely to have had any episode of physical disability.” LIFE’s researchers say the results could have been more dramatic but for the fact that those in the workshop group learned about the benefits of walking and some started to walk and exercise on their own.  

The advantages of walking include stopping bone mass loss, strengthening leg and abdominal muscles, and improving circulation by helping to lower blood pressure and reducing the risk for heart disease. It helps people lose weight or maintain a healthy weight, sleep better, increase mental sharpness, and improve balance and stability. Doctors say walking can increase your lifespan but if you have not exercised in a while, they caution to slowly increase your pace as well as the distance you walk. If you are unsteady on your feet, use a cane or walker, and everyone should wear properly fitting footwear.

The goal of a snack should be to satisfy your hunger as well as be a balanced addition to your daily nutritional intake. Dietician Lisa Jones says satisfying snacks can be crunchy, creamy, savory or salty and good choices include things like eggs, tofu, whole grains, and fruits and vegetables.

To get 80% of the daily folate and 50% of the manganese requirement, how about eating a cup of shelled edamame?

Other quick snacks include a handful of nuts and a piece of fruit, or a bowl of popcorn topped with a small amount of olive oil rather than butter. Cut-up veggies eaten plain or dipped in peanut butter or humus is another satisfying, healthy snack. Greek yogurt is loaded with calcium, probiotics and protein and can be added to fruit, a handful of nuts or even to tuna.

Research Discoveries Offer Hope and Optimism for Alzheimer’s Patients

Recently John C. Morris, MD, Director of the Knight Alzheimer Disease Research Center at Washington University in St. Louis and member of the ABF research advisory committee; said there are several reasons to have hope and optimism for the future regarding Alzheimer’s disease. Involved in Alzheimer’s research since 1983, he explained for the first time he believes “there is now a great opportunity to develop effective therapies, including drugs currently under evaluation for treatment.” He said advances also include an important tool for diagnosing the disease.

One of the reasons for optimism was the announcement of the pharmaceutical company Biogen’s controversial new drug, which purportedly is designed to treat Alzheimer’s disease and not just the symptoms. The drug, aducanumab, was approved for patient use on June 7, 2021. However, doctors say it is important to know the FDA’s approval is only for the drug’s ability to reduce the amount of amyloid plaques in the brains of people given the drug. They state it is still unknown whether the removal of the plaque offers benefits to the patients. To be eligible to receive the drug, patients need to have a firm diagnosis of Alzheimer’s disease.  

In the year 2020, we also heard the news about a ground-breaking new blood test at the Alzheimer’s Association International Conference 2020. Researchers say the new test identifies the protein ptau-2017, a biomarker for Alzheimer’s, and results have been far more accurate than several other biomarkers currently being tested. This is good news as doctors say earlier testing will lead to earlier interventions. The test received a Breakthrough Device designation by the FDA in October 2021.

The MIND Diet May Help Reduce the Risk or Slow Cognitive Decline

Researchers have found evidence suggesting genetics, environment, and lifestyle can affect our risk of developing Alzheimer’s disease and dementia. A poor diet is one of those lifestyle choices that causes high blood pressure and heart disease, and also increases a person’s risk for Alzheimer’s disease. 

WebMD.com reports the MIND diet; a combination of the Mediterranean and DASH diets has shown promise in reducing the risk or slowing cognitive decline. They cite early studies showing MIND lowered the “risk of Alzheimer's by 53% in those who follow it closely and by 35% in those who follow more loosely.” In 2018, the MIND diet recommendation was expanded to include older adults who have suffered a stroke. 

To adhere to the diet, weekly meals should include food from the following groups: green leafy vegetables, other vegetables, nuts, berries, beans, whole grains, fish, poultry, olive oil, and wine. They say a good way to start is with a daily salad, one other vegetable, and three servings of whole grains every day. In terms of vegetables, collard greens, kale, and spinach have been found to be especially healthy. 

Poultry should be eaten once a week, fish twice a week and beans every other day. Nuts and berries make good snacks, blueberries and strawberries being some of the best. Olive oil not only adds a great taste to foods but has been shown to improve brain function over the long term and protect against dementia.

If you worry about forgetting things, Dr. Vernon Williams, a sports neurologist and founding director of the Center for Sports Neurology and Pain Medicine at the Cedars-Sinai Kerlan-Jobe Institute in Los Angeles, says for most of us it is normal. He states all of us forget things like where we put our keys or when our doctor appointment is, which is typically not a reason for concern. Alzheimer’s disease and other types of dementia are different and cause more consistent memory problems that have a debilitating effect on a person's quality of life.

Along with physical exercise, other strategies to improve and protect memory include challenging our brain by learning new things, sleeping well and eating a healthy diet. Dr. Williams says adults need at least seven to nine hours of sleep a night as it “allows the brain to heal and restore itself, clearing toxins that can lead to Alzheimer's disease and other kinds of dementia.”

Tai chi is one of the best-known martial arts as well as one of the oldest as it was developed around 1670.  It has been used throughout the centuries to help balance the body and mind and recently has gotten positive press for being a good arthritis workout.  In fact, several years ago, a study from the Tufts Medical Center in Boston, Massachusetts, found that “tai chi could specifically reduce the pain and physical impairment of people with severe knee OA.”  In 2013, the Centers for Disease Control and Prevention (CDC) recommended a “Tai Chi for Arthritis for Fall Prevention Program.” 

The Scope, a blog produced by the Office of Communications at Stanford Medicine, published a study in 2018 declaring that tai chi can reduce the risk of falls.  The multi-institutional clinical trial “assessed the effectiveness of two proven exercise interventions — tai chi and a multimodal exercise program — which were compared to a control intervention of stretching.”  

The study consisted of 670 participants aged 70 and older with a high risk for falls based on limited mobility or a history of falling the previous year.  Each was randomly assigned to take part in twice weekly, 60-minute exercise sessions for 24 weeks of tai chi balance training, multimodal exercises, or stretching.  Study results were based on monthly self-reporting from participants on incidences of falls, and then confirmed with follow-up appointments and medical records.  As reported in JAMA, the study found tai chi to be more effective in reducing falls during the six-month period.  The data showed “152 falls occurred among 85 participants in the tai chi group, 218 falls among 112 participants in the multimodal exercise group and 363 falls among 127 participants in the stretching group.”

The Link Between Diet and the Risk for Alzheimer’s Disease

Several studies are showing links to our dietary habits and the risk for Alzheimer’s disease.  The Mayo Clinic reports that a hybrid of two diets, the DASH diet that helps reduce high blood pressure and the Mediterranean diet that improves heart health, may protect our brain health.   

Known as the "MIND" diet, which is short for Mediterranean-DASH Intervention for Neurodegenerative Delay, the diet consists of lots of natural plant-based foods while limiting red meat, saturated fat and sugar.  Based on observational studies, research suggests that following the MIND diet can “reduce the risk of developing Alzheimer's disease by up to 53 percent as well as slow cognitive decline and improve verbal memory.”

One category of food at the top of the list is green leafy vegetables with a recommendation of at least six servings a week in dishes such as salads.  The diet also includes one serving of other vegetables daily.  MIND’s primary fruit is to eat berries at least twice a week.  Red meat is reduced to less than four times a week, cheese to less than one serving per week, while poultry and fish consumed twice or once a week respectively.  Other recommended proteins include nuts and beans.  

Oil should be mainly from olive oil and butter or margarine reduced to less than a tablespoon a day; and fried or fast food eaten less than once a week.  MIND includes a glass of wine a day but limits pastries and sweets to less than five times a week.   The Mayo Clinic says study results also found you do not have to strictly follow the diet to gain benefits.  For example, “adults in the study who followed the diet most closely (an average score of 9.6 points out of 15) saw the biggest drop in their Alzheimer's risk, the ones who scored in the middle (7.5 points) still cut their risk by over a third.”
