Caring for a person with Alzheimer’s disease can be a time-consuming job. As the disease is progressive and symptoms worsen over time, the person’s needs also evolve. Things they used to do for themselves may no longer be possible or safe. Moreover, experts say like many older adults, a person with Alzheimer’s is likely to experience health changes over time. Therefore, it is important he/she continues to get routine checkups with their primary care physician as well as with any specialists they may be seeing.

The National Institute on Aging (NIH) says some of the problems caregivers should look for are signs of fever, or sickness like the flu or pneumonia. They say a fever could be signs of infection, dehydration caused by lack of fluids, heat stroke or even constipation. When taking a person’s temperature, use a digital thermometer rather than a glass one as the person might bite down on the glass and break it. Let their doctor know right away if they do have a fever. To help prevent serious illness from flu or pneumonia, have the person get a yearly flu shot and a pneumonia vaccine if recommended. 

Dehydration can occur in people with Alzheimer’s as they may forget to drink fluids throughout the day. Caregivers should look for signs of dry mouth, dizziness and rapid heart rate. Experts say dehydration can even cause hallucinations; however, they are also a symptom of Alzheimer’s disease. 

As the disease progresses, the person will most likely need help brushing their teeth. It is important to look for any sores in the mouth or oblivious signs of dental problems. 

NIH cautions caregivers to be on the lookout for signs of pain as the person may not have the ability to tell you. They recommend watching the person’s face for outward signs of pain, or sudden changes in their behavior such as yelling or striking out. If you are not sure what is happening, call their doctor for help.

To help those dealing with end-of-life issues, the National Institute on Aging (NIA) explains some of the myths associated with palliative and hospice care including sorting out the difference between the two terms. They explain while they are similar, they do differ in a couple of significant ways.

Palliative care is defined as providing specialized care for those living with a serious illness. Myths linked to palliative care include the person no longer being able to receive care for their disease. The fact is care can be provided for the illness along with curative care. Another myth is the person will no longer be able to see their primary doctor but will only be treated by specialists. NIA says in many cases, the person’s primary provider will work hand-in-hand with specialists.

The intention of palliative care is to improve quality of life as well as provide care to improve help with symptoms. It also helps the patient understand their choices for medical treatment. A palliative care team may be made up of specialized nurses and doctors along with social workers, nutritionists and chaplains. If a time comes when the person’s care team decides treatment is no longer working, palliative care could transition to hospice care if the doctor believes the person is likely to die within six months.  Another option is the palliative team would continue to help with the emphasis on providing comfort care. 

The NIA explains hospice care provides comfort and quality of life for a person with a serious illness who is facing end of life. The myth surrounding hospice care is the person cannot receive any medications. The truth is a person will receive medications to help manage symptoms but not to treat the disease. Many people think hospice care can only be done in a hospital or nursing facility but experts say it can also be done at home.

Insights on Longevity from Centenarians

According to recent statistics, there are an estimated 101,000 centenarians (age 100+) making up 0.03% of the population; and experts expect that number to quadruple over the next three decades. Compare that to just 2,300 centenarians estimated to be alive during the 1950 census. Interestingly, Japan has the greatest number of people in their 100s, at 146,000 with the US holding the second spot. China has 60,000, India 48,000 and Thailand 38,000 to round out the top five.

Experts say staying physically active is beneficial for overall fitness as well as extending our lifespan. Many centenarians report they routinely engage in physical activity such as walking, gardening, or participating in community activities. Studies link staying active to maintaining strength, flexibility, and cognitive function, which all contribute to not only a longer life but a healthier one.

To learn the secrets to a long life, Today.com interviewed several centenarians, including some that are still driving, working and even getting married. One 100 year old recommends not retiring if you are happy working. In fact, she reports to work every day at her family’s furniture store. The woman still drives, and once she is home, cooks her meals from scratch which often include vegetables and herbs from her own garden. In terms of diet, she says she doesn’t eat fried or fast food, avoids alcohol and makes getting a good night’s sleep a priority. 

Another 102 year old woman passes along advice that includes the philosophy of having “no regrets,” and recommends not wasting time looking back but rather looking forward to the things you want. She still works three days a week and her health routine includes walking every day and staying positive.   

Others interviewees said to eat lots of fruits and vegetables, and avoid foods high in sugar. There were many who commented on the benefits of a positive attitude, and said they try to bring joy and love into their lives. The centenarians also agreed expressing gratitude and spending time with loved ones is important to a fulfilling, long life.

Balance Begins to Decline in Midlife

Falls among senior citizens are all too common, and in many cases, threaten a person’s independence and way of life. Injuries from falls include to the head, hip fractures, and in extreme cases lead to death; and poor balance is one of the reasons people fall.

Experts explain a person’s balance declines with age due to loss of sensory elements, the ability to integrate information and issue motor commands; and loss of musculoskeletal function as well as poor vision and inner ear problems. Certain medications and common medical conditions in older adults can also cause problems. Balance begins to decline in midlife, starting at about age 50. In one study, adults in their 30s and 40s could stand on one foot for a minute or more. By age 50, the time decreased to 45 seconds.

The National Institute on Aging (NIH) explains maintaining good balance as you age and learning about fall prevention can help you get around, stay independent, and carry out daily activities. Targeted exercises like walking, strength training and workouts designed to improve stability are some of the best ways to improve balance. Consult with your doctor as to whether a physical therapist or other professional could help design suitable exercises to improve your balance. 

Also talk to your doctor about any dietary changes you should make if your balance problems are due to high or low blood pressure. Something as simple as eating less salt, maintaining a healthy weight and exercising may help with high blood pressure. 

If you have low blood pressure and experience dizziness, do not stand up too quickly, drink plenty of fluids such as water and avoid alcohol. Your doctor may need to evaluate if it is safe for you to drive. To reduce your risk of a fall, avoid walking in the dark, wear low-heeled, rubber-soled shoes; and if you need a mobility aid such as a cane or walker, use it.

Ensuring Adequate Vitamin D Intake

There has been much in the news lately about Vitamin D including how much we need each day and the fact that far too many people may be deficient. Medical professionals say there are not many foods high in Vitamin D so relying only on what we eat can be challenging. It is estimated close to half of all Americans are deficient in D; but without a blood test, it is difficult to know for sure. 

Vitamin D is good for us as it contributes to bone health by aiding our bodies with absorbing calcium and phosphorus. It additionally helps nerves carry messages between the brain and other parts of the body as well as fights inflammation.  

One food high in D is rainbow trout. Dieticians say one 3-ounce serving contains 645 IUs, which is more than the daily recommended serving for adults. Other foods high in D include salmon, which is high in vitamin D and omega-3 fatty acids, egg yolks and canned tuna. Mushrooms grown under UV light also have a good amount of vitamin D. Another source is food fortified with D such as many types of milk, yogurt, cereals and orange juice. 

If you are concerned about sun exposure due to the risk of skin cancer and don’t want to get vitamin D that way, experts say it only takes about 10 to 30 minutes of daily direct sunlight exposure to ensure vitamin production. The National Institutes of Health recommends a daily allowance of 600 international units IU (15 micrograms) of vitamin D for most adults. For seniors 70 years and older, 800 IU (20 micrograms) is the daily recommendation. Ask your doctor for a blood test to determine your current levels and how much vitamin D you need.

Survey of Pharmacists Highlights Myths About OTC Medications

Medical professionals worry far too many people do not use over-the-counter (OTC) medications according to stated directions. The most commonly used OTC medications are for pain management, allergy treatments and to reduce the symptoms of flu. 

US News & World Report conducted a survey of more than 350 pharmacists nationwide from chain, independent and hospital pharmacies regarding their views on patients’ compliance with OTC drugs. The results showed 9 out of 10 pharmacists agreed consumers believe too many myths about OTC medications.

The list of some of the most commonly-held misconceptions includes:

  • It’s okay for children to take adult OTC medications as long as the doses are reduced.

  • It’s okay to double-dose OTC medications when I forget a dose.

  • Natural or organic supplements are safer than other OTC medications.

  • The bathroom is a good place to store OTC medications.

  • It’s okay to take OTC medications after their expiration dates.

  • It’s okay to cut, crush or chew OTC pills.

  • Brand name OTC medications or products are clearly better than their generic versions.

Debunking the first item, experts explain it is not okay to give infants and young children adult medications as they need smaller doses that are best measured using appropriately designated formulations. They remind the public there are many medications designed specifically for children. Hanna Phan, clinical associate professor at the University of Michigan College of Pharmacy in Ann Arbor, Michigan, says, “Substituting adult forms, which are often tablets or capsules, can be inaccurate and may increase risk for medication errors, such as overdose.” 

Other myth busting information is to store OTC drugs in a dry, cool spot as recommended for prescription medications. Pharmacists do say it is probably okay to take an OTC a few months after the expiration date; but much longer and the active ingredients may have lost their effectiveness. Also, they say for the most part generic versions of a product are as good as a brand name one. Another warning is to not double up on a medication even when you miss a dose. Moreover, just because something is labeled as organic doesn’t mean it is always safe for you to take. Rather check with your doctor.

Overview of the Four Parts of Medicare

If you recently had a medical procedure or were hospitalized, you may be confused about a recent bill and are wondering what Medicare will cover, what your gap insurance will cover, and what you will be responsible for. It may help to have a better understanding of the four main parts of Medicare.

Part A helps pay for inpatient stays in hospitals and skilled nursing facilities, some home care and end-of-life hospice care that accept Medicare. Hospital services covered include a semi-private room and meals, general nursing, drugs and supplies, inpatient rehabilitation, and some blood transfusions. Skilled nursing services include care for things like recovering from hip replacement surgery or stroke. Part A charges a deductible for hospital stays (the fee for 2025 is $1,676) but your Medigap insurance may cover that expense. Hospital stays for up to 60 days and virtually most services are covered by Medicare; but the one exception is not in a private room.  

Part B covers doctors and outpatient services, and together with part A make up what is considered “Original Medicare.” Part B pays for diagnostic screenings, lab tests, preventive services, some medical equipment and transportation. It covers doctor visits and other outpatient services. There is a monthly premium with Part B, and in 2025 it is $185. For those making over $106,000 annually it may be higher. Part B only pays a portion of covered expenses and Medigap insurance policies are designed to cover some or all the difference depending on the plan.  

Part C is what Medicare Advantage comes under, and is privately held insurance rather than Medicare, which is run by the federal government. If you choose Medicare Advantage, you will still need to sign up for Medicare part A and B and usually pay the premium for part B. Medicare Advantage plans set their own deductibles and you typically need to stay in a Network and use your plan’s approved providers or pay higher out of pocket costs. 

Part D is again from a private insurer and pays for some of your prescription drugs. Generally there is a co-pay for each prescription, and you may have a premium, and a yearly deductible. Check with Medicare regarding rules for higher yearly drug costs and your responsibility.

Remember Medicare offers an open enrollment period each year to allow recipients to make changes to their plan. The period starts on October 15 and ends December 7. Any changes you make during this time will go into effect the following year.

Age Wave, a company specializing in issues related to the aging population, surveyed how older adults feel about senior living. It examined the decision-making process including priorities, hopes, concerns, and questions of those surveyed as well as conducted interviews with residents and management in senior housing.  

The first topic the survey looked at is whether a senior’s current home is in reality the best place to retire in. Age Wave said results show a move to senior living can in many cases provide more freedom, more convenience, better care and less worry for seniors. 

One sentiment expressed by many in senior living communities is that they appreciate the fact that if their health needs change, or those of their spouse, the community is equipped to deal with the needs without necessitating a move. Also, if they need to see a doctor, they like that they can just schedule a ride to their appointment. 

Another myth is that people fear they will no longer have purpose in their life in a senior community. However, according to the Age Wave survey, people in CCRC reported at this time of life perhaps there is a better purpose than doing household chores. Residents expressed their satisfaction in the freedom to explore new pathways – “from learning how to paint, volunteering in the community, becoming an expert on healthy cooking, authoring a book, reaching new levels of fitness, to developing rewarding new friendships.” 

Sometimes, seeing the costs of senior living, people worry it is too expensive. Again Age Wave says several misconceptions can lead seniors to miscalculate the cost of staying in their home to community living. Adopt Incremental Changes for a Healthier Lifestyle

Medical experts from the Harvard School of Medicine explain that even small steps can make a positive change in your wellness journey. Many of their recommendations can be done daily without much time or effort.

The first tip is to start the morning with a full body stretch to help improve circulation and wake up the body. Before getting out of bed, flex your lower limbs several times. Then bend your knees and raise your legs. Flex and rotate each foot a couple of times.  From a sitting position, turn your head from side to side a few times and rotate your shoulders. Extend your arms out and rotate your hands.   

To maintain your dental health, floss your teeth every day. Experts say the proper way to floss is to wrap the floss around each tooth and slide it up and down a couple of times against the tooth rather than using a sawing motion. Now do the tooth on the other side. Another easy healthy habit is to drink a glass of water when you get up each morning, and then drink one with each meal. Apply a sunscreen of at least an SPF 30 each morning after you wash your face to help prevent skin cancer. 

As you can see, staying healthy doesn’t always require a lot of time. A few quick physical tricks include every time you stand up, sit right back down and do it again. Think about your posture and try to put your shoulders back and stand up straight. When you brush your teeth, suck in your abdomen and hold for a few seconds. If you have been sitting for a while, take a quick walk around or incorporate a dance step into your movements. Doing a few pushups or squats is also a good way to wake up your body.  

Research shows socializing impacts a person’s overall wellness so make a point to get out and spend time with your family and friends. Get in the habit of cultivating new interests or hobbies. Joining things like a book club or cooking class can be both mentally stimulating and socially invigorating.

Mental Exercise is Key to Brain Health

We all know that even as seniors, physical exercise is important to maintain healthy bodies but medical experts want us to remember that mental exercise is also essential to good brain health.  Luckily there are many fun and entertaining ways to exercise our mind and include an array of games on our tablets or computers as well as ones that only need pen and paper.  Games like Sudoku, mahjong, chess, crossword puzzles, jigsaw puzzles, and solitaire are enjoyable and challenging ways to improve memory and stimulate the brain.    

Moreover, there is good evidence that supports this claim such as the study from the Albert Einstein College of Medicine.  The research team conducted a long-term study that found seniors who did crosswords four days a week had a 47 percent lower risk of dementia than those who only did them once a week.  

Also, while there is the perception that chess is only for those who are very intelligent, it turns out the skills used to play a good game of chess are ones we all should cultivate as they may make us smarter.  Experts say chess encourages “concentration and focus around ‘if then’ thinking, which is useful when making decisions.”  The game forces you to think a few steps ahead and that type of thinking can be especially helpful for older people who are navigating options in life and could use a little help with creative thinking.

Why Glaucoma Screening is Important

Statistics show glaucoma is the second leading cause of blindness worldwide, and it is important to know that in the early stages, there are usually no symptoms. This is why Medicare says half of the people who have the disease do not know they have it. 

The best way for someone to know if they have glaucoma is to get screened for it through a dilated eye exam with visual field testing. For seniors on Medicare, the program covers one glaucoma screening each year for individuals at high risk. 

The National Eye Institute wants the public to know early treatment of the disease can stop the damage and protect your eyes from serious vision loss. People most at risk for glaucoma include those with diabetes, a family history, African Americans over the age of 50 and Hispanics aged 65 or older. 

As stated earlier, glaucoma in the first stages does not generally exhibit symptoms for most people. Later on, symptoms include the loss of side (peripheral) vision and blind spots, and without treatment blindness. Since symptoms start so slowly, many people do not notice the change to their vision. 

The cause of glaucoma is not known but it is linked to high pressure in the eyes; and one treatment is to lower the eye pressure through medication. While one or both eyes can be affected, some people with high eye pressure do not have glaucoma. There are several different types of the disease with open-angle glaucoma being the most common in the United States. Treatments include eye drops, lasers and surgery.

Adding Oranges to Your Diet is Linked to a Lower Risk of Depression

An article from the Medical News Today reports oranges may be the new apple a day remedy.  They mention a Harvard study showing a daily orange may do more than just provide a juicy dose of vitamin C — it could also help protect our mental health.

The study found a connection between consuming one medium orange daily with a 20 percent decrease in the risk of developing depression. Researchers say the lower risk is due to the effects oranges, and other citrus fruits, have on the beneficial gut bacteria called Faecalibacterium prausnitzii. Participants who reported feeling depressed were found to have lower levels of this bacterium. 

Researchers say the bacterium seems to influence the production of the brain chemicals serotonin and dopamine, which are associated with regulating our mood. The large study tracked 32,000 middle-aged women from 2003 to 2017; and throughout the extensive time frame, discovered only citrus fruits seemed to help stave off depression. A later study found similar results in men. 

As an observational study showing a correlation, it doesn’t directly prove that eating oranges can prevent depression. Rather what it does is add to the growing amount of data we have regarding the connection between gut health and our mental well-being. In addition to the possible benefits to our brain health, the vitamin C in citrus is essential for healthy immune systems as well as helps the body produce collagen and maintain healthy blood vessels and heart.

The Medical News Today writes, “Diabetes — specifically type 2 diabetes— is becoming one of the most challenging health problems of the 21st century. They add that beyond dealing with the serious disease, it is a risk factor for other health conditions such as cardiovascular disease, renal disease, stroke and blindness among others. 

Preventing prediabetes from progressing to type 2 diabetes is important, and experts suggest that there are measures individuals can take to improve their health. Many people with prediabetes do not exhibit symptoms, but some indicators to watch for include frequent thirst, nighttime bathroom trips, and fluctuations in energy levels. 

A high BMI or obesity puts people at risk for prediabetes or diabetes as does aging. Insulin becomes slightly less effective, and its receptors and beta cells do not function like they once did when we were younger. Another risk factor is lack of exercise and a diet too high in high glycemic or sugary foods. It is also important to factor in a family history of diabetes as it is a genetic condition.  

Dr. Barber, an associate clinical professor at Warwick Medical School and consultant endocrinologist at University Hospitals Coventry and Warwickshire, explains, “We know that intensive lifestyle treatments with focus on diet, physical activity, weight loss, and so on, can really help in preventing, or at least delaying the onset of type two diabetes.” If you suspect prediabetes or have been told you already have prediabetes, it is important to work with your healthcare provider to make the lifestyle changes needed to improve your health.

A Change in Walking Gait May Be an Indicator of Alzheimer's Disease

Several studies, including one from the journal Current Biology, report the way a senior walks may be messaging something about their future cognitive health. Researchers say walking relies on a person’s navigational skills, which demands mental engagement. 

The slowing of a person’s walking pace is one of the things studies are looking at as an early warning sign of Alzheimer’s disease. People may start to take smaller steps and drift from side to side as well as exhibit poor balance and a history of frequent falls.  Ryan Glatt, a certified personal trainer, senior brain health coach, and director of the FitBrain Program at Pacific Neuroscience Institute in California, told Medical News Today, “In clinical settings, gait-based tests can be helpful for determining fall risk, especially with distractions, and perhaps can be part of a more comprehensive cognitive evaluation in the near future.”

Other studies report that people with early Alzheimer’s disease have difficulty turning when walking. Researchers say the problem was not detected in older, healthy participants who suffered from mild cognitive impairment, suggesting the problem is specific to Alzheimer’s. Glatt explained the reason might be connected to changes in the hippocampus, a region of the brain partially responsible for spatial memory that helps to control visuospatial navigation.

Another early warning sign of Alzheimer’s is an altered sense of a person’s environmental perceptions. For example, a person may struggle to understand objects, and become confused by thinking something is 2D when it is really 3D; or vice versa.  

Dr. Jonathan Fellows, who leads the Michigan Institute for Neurological Disorders, Alzheimer’s Disease and Memory Disorder Center, adds, “Observation of gait is an important assessment in the diagnosis of various dementias, including Alzheimer disease, as patients with this disorder can have what is called dyspraxia, or difficulty doing a previously learned task – in this case walking or turning.” Researchers explained that some of the problems that appear to be specific to Alzheimer’s might allow doctors to diagnose patients earlier; and without the need for blood, spinal fluid or speech tests.
