How to Clearly Communicate Symptoms to Your Doctor

Although it is first-hand, it is not always easy to describe to a doctor what is going on regarding medical problems. Describing various symptoms is quite different from getting the results of actual lab tests or a doctor’s opinion after a physical exam.  

Relating symptoms clearly can help narrow down the problem, and medical experts share some advice on how best to do it. They start by telling patients to skip the medical jargon and just use their own words to describe what they are feeling. 

Dr. Lisa Ravindra, an assistant professor in the department of internal medicine and a primary care physician at Rush University Medical Center in Chicago, says, “Avoid using vague terms for timing like ‘for a while’ or stating that a symptom occurs ‘once in a while’ since these mean different things to different people.” She explains it is helpful to be as precise as possible when talking about symptoms including the amount of time you have had them for such as three days, or on and off for a week. Doctors say it is helpful to rate your pain from one to ten. 

Other helpful things you can do include using a blood pressure monitor to chart your blood pressure, and if you feel feverish, take your temperature. Keep track of other important symptoms such as sore throat, earache, coughing, vomiting, diarrhea, etc. Be sure to mention unexpected weight loss, fatigue when doing things you typically do, or specific type and location of pain, i.e. a constant, sharp pain in the front of the right knee. If you have a chronic condition, doctors say it helps to keep a journal of symptoms to discuss with your doctor during your next visit.

Hearing Loss May Contribute to Social Isolation, Depression and Dementia

According to the Medical News Today website, studies find hearing loss may contribute to social isolation, lower quality of life, disability, depression, and dementia. This can be especially problematic for people aged 80 years and older, who tend to have more advanced hearing losses. 

In 2015, researchers reported on a 25-year study, which focused on a link between hearing loss and dementia. At the start of the study, the 3,670 participants were 65 years or older and followed for over 25 years. Using the Mini-Mental State Examination (MMSE) to measure cognitive decline, researchers say there appeared to be a correlation between self-reported hearing loss and cognitive decline. However, participants who used hearing aids did not show a significant cognitive decline, suggesting the use of hearing aids may offer a protective effect.

Experts say hearing loss may adversely impact a person’s ability to socialize as they miss out on vital verbal and emotional information that may directly impair brain structure and function. Or it could be that the person uses more energy to make sense of what they hear; and reduces the energy they have left for memory, language processing and attention.

It is recommended at age 60, all adults get a baseline hearing test and be rechecked every couple of years, regardless of whether they experience symptoms. John Hopkins Medicine reports one in 3 adults over age 65 has hearing loss, but due to a gradual change in their hearing, they may not be aware of it.  

Symptoms of age-related hearing loss, or presbycusis, include the speech of others sounding muffled or slurred; high-pitched sounds, such as “s” or “th” being hard to distinguish; and having difficulty understanding conversation, especially if there is background noise. The person may notice men’s voices are easier to hear than women’s, because it is lower-pitched.

Furthermore, harmful errors in primary and outpatient health care settings are typically related to diagnoses, prescriptions and use of medicines.  Mistakes associated with misdiagnosis result in delayed treatment for the real problem and having the wrong medications prescribed or supplied can cause serious complications.  

However, it is important to keep in mind that some medical errors are a result of patients failing to disclose all the medications they take including prescription and over-the-counter drugs.  Another problem is the failure to reveal pertinent details regarding a person’s prior medical history as well as lifestyle habits such as smoking, diet and alcohol consumption.

Late Life Depression

Depression can affect people at any age including senior citizens.  According to Web MD, “Late-life depression affects about 6 million Americans age 65 and older, but only 10% receive treatment for depression.”  They suggest the reason for this may be that the symptoms seniors display are often ignored or assumed to be a result of illnesses and medications or just a normal part of growing older. 

There are many reasons why seniors suffer from depression including major changes in their lives such as retirement, the death of a spouse, illnesses, disabilities and social isolation.  People suffering from very painful, disabling, or life-threatening illnesses are especially vulnerable to depression or the worsening of symptoms.  Furthermore, depression can be caused by medications that are commonly prescribed to the elderly including blood pressure medication, beta-blockers, sleeping pills, tranquilizers, calcium-channel blockers and medication for Parkinson’s as well as others.

Suffering from depression prevents seniors from enjoying life as they used to and can be the cause of a change in appetite, problems sleeping and impact physical health.  HelpGuide.org says, while sadness is one sign of depression, some seniors do not recognize or admit to feelings of sadness.  Rather someone may “complain instead of low motivation, a lack of energy, or physical problems.  In fact, physical complaints, such as arthritis pain or worsening headaches, are often the predominant symptom of depression in the elderly.”

All too often seniors just accept depression as a part of the aging process but there are steps you can take to help yourself or your loved one.  You first need to talk to your health care provider to rule out other problems, and then perhaps you need to make changes in your lifestyle.  Experts suggest that isolation and inactivity can make depression worse and they recommend getting involved in physical, mental and social activities.

Exercise For Your Bone Health

The Bone Health & Osteoporosis Foundation reports approximately 10 million people in the United States have osteoporosis, and 44 million have low bone density, meaning they are at risk of developing the condition. One in two females and up to one in four males will be affected by a broken bone in their lifetime due to osteoporosis. Medication, calcium, and vitamin D and K can help strengthen bones. 

In addition, if you are diagnosed with osteoporosis or low bone mass, an important lifestyle habit is to introduce weight-bearing exercises into your daily routine. The Harvard Medical School publication explains even if you have already had a fracture, you should be staying active. They explain the risk of another fracture is very high, so you want to do everything you can to lessen the chances of that happening by increasing your bone density. 

Weight-bearing exercises can reduce the risk of a fall as well as reduce the risk of fracturing a bone if you do fall. The publication cites an analysis reported on by the British Medical Association’s BMJ, which found that programs of balance, strength, and resistance training reduced the odds of falls resulting in fractures by more than 60%. 

Harvard Medical School says the four components of an exercise program for osteoporosis should include:

  • Weight-bearing exercises that force your body to work against gravity. These include walking, climbing stairs, playing tennis, and dancing. (Higher-impact activities strengthen bone more than lower-impact exercises, but only do what your fitness level allows).

  • Muscle-strengthening exercises using weights or your body’s own resistance to work against gravity. This includes lifting free weights, using a weight machine, working with resistance bands, and lifting your own body weight. Do it at least twice a week.

  • Balance exercises can improve your ability to hold yourself upright and help prevent falls. Examples of these types of balance exercises are tai chi and yoga. Again try to do these at least twice a week. 

  • Flexibility exercises are useful to ensure muscles stay limber, and joints mobile.  Stretching and yoga are good exercises. Experts recommend stretching after every workout for five to 10 minutes. 

A physical therapist or certified fitness trainer can help ensure you are doing your weight-bearing exercises safely. Experts say start slowly and build from there, and remember to be gentle in regard to your condition.

Monitoring Your Blood Pressure at Home

Many of us have had our blood pressure taken at the doctor’s office and were surprised when it turned out to be higher than we expected it to be. If this has happened to you, you may be experiencing something known as “white-coat hypertension,” meaning a higher than warranted reading during a medical appointment. 

Doctors say one of the best ways to ensure you do not have elevated blood pressure, is to take your blood pressure at home a number of times. You may also be asked by your healthcare provider to track your blood pressure to see if you should be put on medication or adjust medication to reach a target number. 

It is important to do it correctly to ensure readings are accurate. Experts say start by buying a validated monitor and do not buy one that uses the finger or wrist as they are not as accurate as upper-arm cuffs. If you have a much smaller or larger upper-arm, look for one appropriately sized. 

Other things to consider are to not take your blood pressure when you are stressed and to wait at least 30 minutes after eating, smoking or exercising. Odd as it sounds you will get a more accurate reading if you empty your bladder right beforehand. Sit comfortably with feet flat on the floor and support your arm near heart height. Now wait a few minutes. A good strategy for a true representation of BP is to take 4 readings a day for 7 days – twice in the morning and twice at night. Let your doctor know your findings.

Taking Your Blood Pressure at Home

Many of us have had our blood pressure taken at the doctor’s office and were surprised when it turned out to be higher than we expected it to be. If this has happened to you, you may be experiencing something known as “white-coat hypertension,” meaning a higher than warranted reading during a medical appointment. 

Doctors say one of the best ways to ensure you do not have elevated blood pressure, is to take your blood pressure at home a number of times. You may also be asked by your healthcare provider to track your blood pressure to see if you should be put on medication or adjust medication to reach a target number. 

It is important to do it correctly to ensure readings are accurate. Experts say start by buying a validated monitor and do not buy one that uses the finger or wrist as they are not as accurate as upper-arm cuffs. If you have a much smaller or larger upper-arm, look for one appropriately sized. 

Other things to consider are to not take your blood pressure when you are stressed and to wait at least 30 minutes after eating, smoking or exercising. Odd as it sounds you will get a more accurate reading if you empty your bladder right beforehand. Sit comfortably with feet flat on the floor and support your arm near heart height. Now wait a few minutes. A good strategy for a true representation of BP is to take 4 readings a day for 7 days – twice in the morning and twice at night. Let your doctor know your findings.

Researchers Report a Link Between Gum Disease and Alzheimer’s Disease

Researchers at Tuft University reported a link between gum disease, also known as periodontitis and periodontal disease, to an increased risk for Alzheimer’s disease. Periodontitis is a bacterial infection within the soft tissue of the mouth, and “about 47.2% of U.S. adults over 30 and 70% over 65 have gum disease,” according to the Centers for Disease Control and Prevention (CDC). 

The study, which was done with mice, discovered the correlation, but researchers hope the findings can be used to someday help reduce cases of Alzheimer’s disease. The authors cite Fusobacterium nucleatum (F. nucleatum), a bacterium that causes gum disease and is already linked to kidney disease, diabetes and coronary problems to be the problem. 

The lead author, Dr. Jake Jinkun Chen, professor of periodontology and director of the Division of Oral Biology at Tufts University School of Dental Medicine explains the purpose of the study is to identify and characterize the association of induced periodontal disease with Alzheimer’s disease. He says as F. nucleatum generates “inflammation both locally and systemically, it affects the Alzheimer’s disease phenotype, as Alzheimer’s itself is an inflammatory disease.” Additionally, since the bacteria migrate and penetrate the brain, it can worsen the symptoms and signs of Alzheimer’s disease. 

Researchers plan further studies in humans and say although there is not a proven link yet; good oral health benefits our overall health. They also say to decrease our risk for Alzheimer’s, it is important to maintain a healthy diet and exercise.

Flavonoids in Foods May Help Forgetfulness Many Seniors Experience

Research shows flavonoids, the chemicals that provide plant foods their bright colors, may help in the fight against forgetfulness and mild confusion many seniors experience. The evidence comes from two long-term continuing health studies from the late 1970s and early 1980s. Participants were periodically surveyed regarding their diet and state of their health including memory problems over a 20-year period. Data was extracted from over 100,000 men and women with an average age of 73.  

The study’s lead author, Dr. Deborah Blacker, a professor of epidemiology at the Harvard T.H. Chan School of Public Health, said they focused on participants’ consummation of two dozen common types of flavonoids. These included carrots (high in beta carotene), strawberries (containing flavone) and apples (containing anthocyanin). Results suggest those who answered “yes” to having memory problems also reported eating a lower number of flavonoid-enriched foods.  

Researchers found a significant difference between the bottom one-fifth of participants who reported the lowest intake of flavonoids compared to the upper one-fifth who were 19 percent less likely to report forgetfulness or confusion. Additionally, the study reported “higher intakes of Brussel sprouts, strawberries, winter squash and raw spinach were most highly associated with better scores on the test of subjective cognitive decline.” Dr. Thomas M. Holland, a researcher at the Rush Institute for Healthy Aging who was not involved in the study, said “There’s some really good data here with 20 years of follow-up.”

Using Prescription Drugs Safely

It may surprise you to know that according to the Department of Health and Human Services, each year fifty-five percent of seniors are non-compliant with prescription drug orders and do not take medication as prescribed by their doctors. It is a serious problem as approximately 200,000 older adults are hospitalized annually due to adverse drug reactions. 

Medical professionals say there are steps seniors and those who care for an elderly loved one can do to help lessen the problem. To start with, take medications as prescribed including the correct dosage and time, with or without food, and never crush a pill into a powder. Especially if you see multiple doctors, be sure to always bring a list and dosage of all medications you are taking including over-the-counter and herbal supplements. 

Experts say using the same pharmacy can help reduce drug interactions, and to be sure to read the warnings regarding adverse reactions. Periodically review your prescriptions with your doctors to make sure you are taking the right dosage and still need the medications. To see if any meds you take have a warning for usage in the elderly, check the American Geriatric Society’s Beers Criteria for Potentially Inappropriate Medication Use in Older Adults. It is also recommended you properly dispose of older medications and drugs you no longer take.  

If you are on medications to treat diabetes or high blood pressure, you need to continue to test your glucose level and BP numbers. Other drugs may be prescribed with the understanding you will have your blood tested periodically and it is important to follow through as too high or too low of a dosage can cause problems.

Good Hydration Is Important for Good Health

Dehydration is a real concern especially for the elderly, or for anyone who spends time in the sun. The Centers for Disease Control & Prevention says over 43% of adults do not drink enough water each day. Although the amount of fluid we need differs, Heather Mangieri, a registered dietitian based in Imperial, PA, says on average men should consume 15 cups of fluid a day and women 11 cups.

Experts say water is the best way to hydrate but for those who find it a chore to drink, there are other healthy alternatives. In fact, statistics show 22% of our daily fluid intake is from foods such as fruits and vegetables, which are especially high in fluid. Some of the most moisture-providing vegetables are lettuce and cucumbers containing up to 96% water. Celery, radishes, spinach and cauliflower are other good choices. Some of the most hydrating fruits are tomatoes, strawberries, watermelon and cantaloupe.

Despite popular folklore, coffee doesn’t cause fluid loss and does provide hydration, but experts say it is still important to limit your amount of caffeine especially toward evening. Drinking too much alcohol does dehydrate the body and one tip to lessen the effects is to consume a beverage high in electrolytes like coconut water. Cold soups are another way to keep your fluid levels high. Oatmeal with berries is a good choice for breakfast as the oats soak up the water they are cooked in and the berries themselves are hydrating.

When an older adult trips and falls, it can be life changing. Consequences include serious head injuries, hip fractures and broken bones, and in the worst cases, fatalities. The Centers for Disease Control and Prevention (CDC) reports 3 million seniors are treated in emergency departments for fall injuries each year. Furthermore, more problems can arise as many seniors need several months to recover, which can cause a decline in strength and mobility as well as mental agility.  It can also lead to the loss of independence and isolation. 

Statistics show most falls happen for a handful of reasons and being forewarned may help prevent one. The environment, especially the person’s home and surrounding outdoor area is one of the biggest culprits. Danger spots include slippery bathtubs, high mattresses, steep stairs, poorly lit hallways, loose rugs, obstacles in walkways and clutter. Experts say a little bit of time spent addressing these problems with things like non-slip bath mats and grab bars, more lighting, removing clutter, etc. can help prevent a fall. 

Another risk factor for a fall is chronic health conditions that affect balance and mobility. Arthritis, heart disease, diabetes and high blood pressure can mean a person might experience dizziness or be unable to control muscles to stop a fall. 

Growing older also puts us at risk for diseases like glaucoma, age-related macular degeneration, diabetic retinopathy and cataracts, which in turn, leads to more accidents. Solutions include having yearly eye exams and making sure your eyeglass prescription is up to date.

Experts say it is important for seniors to stay as fit as possible as aging affects muscle strength, flexibility and coordination. Check with your doctor for information on balance and strength exercises. If a medication is making you dizzy or drowsy, talk to your doctor about it. There may be another drug just as effective and doesn’t have the same side effects.
