Buying Over-the-Counter Hearing Aids

Did you know the FDA has made changes to regulations regarding hearing aids, which now makes it possible to buy an FDA approved device over the counter (OTC)? While many of us have heard about the high cost of prescription hearing aids and that Medicare typically does not cover them, you may want to know whether the experts think an OTC purchase is a good idea. 

The Harvard Medical School went to Dr. James Naples, assistant professor of otolaryngology/head and neck surgery at Harvard-affiliated Beth Israel Deaconess Medical Center, to see if it is a viable option and to explain potential pros and cons. He explains that despite there being several types of hearing aids, they all amplify sound and use a combination of signaling processing and directional microphones to filter out background noise. However, he warns against confusing personal sound amplification products (PSAPs) with hearing aids as they only amplify sound. 

To determine your level of hearing loss, experts recommend you start by getting a hearing test from an audiologist like you would get an exam from an optometrist when buying eyeglasses. This is especially necessary for severe hearing loss, as OTC devices are not appropriate for all types of hearing loss. For mild to moderate hearing loss, it may be possible to get good results by purchasing an OTC device without a hearing exam and fitting by an audiologist. 

The next question routinely asked is if you will save money by purchasing an OTC hearing aid and the answer is most-likely yes as a single hearing aid’s average cost is $2000, and age-related hearing loss may be in both ears. OTC also helps reduce the price by eliminating the added expense of the hearing test along with the actual device being substantially lower. Doctors say the new regulations should increase competition and some estimates indicate prices could drop by as much as $1,600. 

Dr. Naples cautions that just as drugstore readers are a great option for many, some individuals have more complicated issues that require more precise solutions. Hearing aids are the same. The doctor states the safest bet is to get a hearing test to confirm an OTC device is right for you.

Taking Your Blood Pressure at Home

Many of us have had our blood pressure taken at the doctor’s office and were surprised when it turned out to be higher than we expected it to be. If this has happened to you, you may be experiencing something known as “white-coat hypertension,” meaning a higher than warranted reading during a medical appointment. 

Doctors say one of the best ways to ensure you do not have elevated blood pressure, is to take your blood pressure at home a number of times. You may also be asked by your healthcare provider to track your blood pressure to see if you should be put on medication or adjust medication to reach a target number. 

It is important to do it correctly to ensure readings are accurate. Experts say start by buying a validated monitor and do not buy one that uses the finger or wrist as they are not as accurate as upper-arm cuffs. If you have a much smaller or larger upper-arm, look for one appropriately sized. 

Other things to consider are to not take your blood pressure when you are stressed and to wait at least 30 minutes after eating, smoking or exercising. Odd as it sounds you will get a more accurate reading if you empty your bladder right beforehand. Sit comfortably with feet flat on the floor and support your arm near heart height. Now wait a few minutes. A good strategy for a true representation of BP is to take 4 readings a day for 7 days – twice in the morning and twice at night. Let your doctor know your findings.

Researchers Report a Link Between Gum Disease and Alzheimer’s Disease

Researchers at Tuft University reported a link between gum disease, also known as periodontitis and periodontal disease, to an increased risk for Alzheimer’s disease. Periodontitis is a bacterial infection within the soft tissue of the mouth, and “about 47.2% of U.S. adults over 30 and 70% over 65 have gum disease,” according to the Centers for Disease Control and Prevention (CDC). 

The study, which was done with mice, discovered the correlation, but researchers hope the findings can be used to someday help reduce cases of Alzheimer’s disease. The authors cite Fusobacterium nucleatum (F. nucleatum), a bacterium that causes gum disease and is already linked to kidney disease, diabetes and coronary problems to be the problem. 

The lead author, Dr. Jake Jinkun Chen, professor of periodontology and director of the Division of Oral Biology at Tufts University School of Dental Medicine explains the purpose of the study is to identify and characterize the association of induced periodontal disease with Alzheimer’s disease. He says as F. nucleatum generates “inflammation both locally and systemically, it affects the Alzheimer’s disease phenotype, as Alzheimer’s itself is an inflammatory disease.” Additionally, since the bacteria migrate and penetrate the brain, it can worsen the symptoms and signs of Alzheimer’s disease. 

Researchers plan further studies in humans and say although there is not a proven link yet; good oral health benefits our overall health. They also say to decrease our risk for Alzheimer’s, it is important to maintain a healthy diet and exercise.

Are You Worried About Your Memory?

Do you feel as if memory problems are interfering with your life or have family members started to notice? Medical experts say there may be a distinction between a person noticing memory loss and others becoming aware of it as this could signal a bigger problem and be an early sign of dementia.  

The Harvard Medical School explains the best place to start understanding the problem is to talk to your doctor. While they may not have the proper training to do extensive screening for MCI (mild cognitive impairment), they can rule out other things causing your memory loss such as medications you take or a recent head injury. Steps you can take on your own to improve cognitive function include getting enough sleep each night, eating a healthy diet, exercising regularly, and reducing stress.

After your own physician has completed an examination, and nothing appears to explain your memory loss, it is time to see a specialist. They, or your local Alzheimer’s Association chapter or hospital should be able to recommend a behavioral neurologist or a geriatric psychiatrist. He or she will probably start with a series of written, on the computer, or oral tests to “assess memory, reasoning, attention, language, visual functions, motor functions, and social functions (such as empathy and knowing how to behave appropriately in social situations).”

They will check for “abnormalities in muscle strength, coordination, reflexes, senses, eye movement, and the pupils’ reaction to light, which can signal abnormalities in specific areas of the brain.” In addition, the doctor will check for Parkinson’s, a stroke or tumors, and will probably schedule a hearing and vision test. If indicated, you may have advanced testing such as PET scans or your cerebrospinal fluid (CSF) biomarkers checked for signs of Alzheimer’s disease.

The result: heart rate, blood pressure and stress levels immediately drop.” Researchers explain interactions between humans and pets can lower cholesterol levels, fight depression and may even help protect against heart disease and stroke.

A Healthy Diet Helps Ensure Your Immune System Functions Smoothly

Over the last several years, we have seen firsthand the benefit of having a robust immune system, which is vital to helping resist and combat diseases and illnesses, and a healthy diet plays a role in the proper functioning of our immune system. While food alone doesn’t directly boost the immune system, experts explain “a well-rounded diet can help prevent nutrient deficiencies that can hinder immune function, which in turn helps to ensure our immune system functions smoothly.” 

We all need certain vitamins and minerals for our immune cells to grow and function; and nutrient-dense foods help protect our bodies in the event of an invasion of bacteria, fungi or viruses. Likewise there are foods known to be harmful to the immune system.  

Alcoholic beverages are one of those things. If you do drink, it is recommended limiting intake to 2 drinks or less daily for men and 1 drink or less daily for women. A standard drink of alcohol is 12 oz. of beer, 5 oz. of wine and 1.5 oz. of liquor. Another thing to avoid is caffeinated foods or beverages too close to bedtime; and overall the FDA recommends no more than 400 milligrams of caffeine daily. 

Foods high in sugar and salt should also be avoided as well as fast food and processed foods loaded with empty calories, fat and sodium that offer little in the way of needed vitamins and minerals. Dairy products can be problematic for some individuals and lead to inflammation, which in turn affects the immune system. Experts say another good rule is to avoid snack treats with lots of food additives, colorings, and preservatives, which are associated with inflammation, and weaken the immune system.

Diagnosing Alzheimer’s Disease

While there is not one single test used to definitively diagnose Alzheimer’s disease, doctors typically use a variety of tools to make a diagnosis. Along with the person’s primary care provider, specialists like neurologists, neuropsychologists, geriatricians and geriatric psychiatrists may all be involved in determining if the patient’s problems are Alzheimer’s or caused by other reasons.

The process starts with a medical workup done by a health care provider that includes the person’s history of medical problems as well as any history of psychiatric, cognitive or behavioral problems. It is important to tell the doctor about all current medications including over-the-counter-drugs and supplements the person is taking. Your doctor will also want to know if there is a family history of Alzheimer’s or other cognitive problems. 

A physical exam should include a check of reflexes, blood pressure levels and questions regarding sleep patterns, diet, exercise and other lifestyle habits. A structural brain imaging scan such an MRI or computed tomography is used to rule out damage to the brain from stroke, tumors, head trauma or a build-up of fluid. Blood and urine tests can rule out other reasons for symptoms. The Alzheimer’s Association says common causes of dementia-like symptoms include depression, untreated sleep apnea, delirium, side effects of medications, thyroid problems, certain vitamin deficiencies and excessive alcohol consumption.

The doctor may also use one of several cognitive, functional tests to evaluate the person’s memory, thinking and simple problem-solving skills. Professionals state, “There is an urgent need for a simple, inexpensive, non-invasive and easily available diagnostic tool such as blood tests to diagnose the disease.” Currently researchers are experimenting with a variety of tests to enable them to track changes more easily in the brain to identify the disease. Currently, health professionals do not recommend routine genetic testing for Alzheimer’s disease.

Learn More About Parkinson’s Disease

We often come to think something is true if we hear it stated as if it is a fact. Doctors say this happens in medicine more frequently than we realize. One such disease with lots of misinformation surrounding it is Parkinson’s disease (PD), a neurodegenerative disorder affecting predominantly dopamine-producing (“dopaminergic”) neurons in a specific area of the brain called substantia nigra. Symptoms develop slowly over years and may include tremors (mainly at rest); bradykinesia (slowness of movement); limb rigidity; and gait and balance problems. 

One common misconception regarding PD is it only affects a person’s movement. Doctors explain this is untrue as symptoms can also include: cognitive impairment or dementia, depression and anxiety, sleep dysfunction, pain, apathy, sexual dysfunction, and bowel incontinence. Another thing many people believe is levodopa, one of the most effective treatments for PD, is only effective for a certain amount of time. However, experts say it is not the drug that becomes less potent but the fact that the “key enzyme that converts levodopa to dopamine (aromatic acid decarboxylase or AADC) is predominantly found in the dopamine neurons of the substantia nigra, which are lost during progression of the disease.” 

People should know tremors are not always a symptom of PD, and that some people with PD never get them. In terms of the trajectory of the disease, it varies with individuals. Also, research suggests staying physically active can slow progression and reduce symptoms. 

Although the cause of the disease is not known nor is there a cure for it, there are treatments used to improve symptoms and allow people to have a good quality of life. While current therapies don’t slow or stop PD, researchers are working to identify biomarkers to enable earlier detection and hope to find better treatments.

The Link Between Hearing Loss and Cognitive Impairment

JAMA Neurology added to the growing body of evidence showing a link between hearing loss and cognitive impairment. The study found treating hearing loss with hearing aids may reduce the risk of developing dementia by up to 19%. In fact, researchers say hearing loss may be the biggest treatable factor for dementia. An article in AARP explains what may cause the link between hearing loss and dementia. 

The first theory regards socialization. Scientists say once a person starts having more difficulty being in a group of people and following the conversation, they may avoid such social gatherings. This in turn can lead to isolation, which is problematic as less social engagement as well as less cognitively stimulating activities and depression are risk factors for dementia. 

Secondly, coping with hearing loss means the person is constantly working to sort out sounds and make sense of what is being said. The brain now may have “fewer resources (brain power) to help preserve thinking and memory abilities.” Scientists also believe hearing loss causes the brain to be less stimulated with sound information, which is linked with parts of the brain shrinking and atrophying faster with chronic deprivation.  

While experts agree we still need more studies regarding the link between hearing loss and a higher risk for dementia, they do know that addressing a hearing issue with hearing aids is beneficial for the person’s overall quality of life. Moreover, there are no negative medical issues associated with the devices.

Some Medications Can Increase Your Risk of a Fall

The Center for Disease Control & Prevention (CDC) reports millions of older adults fall each year—resulting in more than 36,000 deaths. Of those falls, 3 million seniors are treated in emergency departments for fall injuries; and one out of every five falls causes an injury, such as broken bones or a head injury. With these alarming statistics, experts say it is important for seniors to know their risk for a fall including whether they are taking medications associated with an increased risk.


The first step is to talk to your doctor to see if you are taking one of these medications, as medications are a common reason for falls. Furthermore, experts say adjusting or making a change in medication is the easiest thing to do to help prevent a fall. If it is determined you are taking a medication known to cause light-headedness, dizziness, sudden drop in blood pressure, or chronically low blood pressure or hypoglycemia (low blood sugar), you and your healthcare team can regularly re-evaluate your need and dosage.

Another way to mitigate your risk for a fall is to do exercises designed to improve leg strength and balance. In addition, be sure to get an annual eye exam and update your glasses as needed. Lastly check your home for trip hazards (keep clutter out of hallways and stairways) and install grab bars and better lighting where needed.
