Good Hydration Is Important for Good Health

Dehydration is a real concern especially for the elderly, or for anyone who spends time in the sun. The Centers for Disease Control & Prevention says over 43% of adults do not drink enough water each day. Although the amount of fluid we need differs, Heather Mangieri, a registered dietitian based in Imperial, PA, says on average men should consume 15 cups of fluid a day and women 11 cups.

Experts say water is the best way to hydrate but for those who find it a chore to drink, there are other healthy alternatives. In fact, statistics show 22% of our daily fluid intake is from foods such as fruits and vegetables, which are especially high in fluid. Some of the most moisture-providing vegetables are lettuce and cucumbers containing up to 96% water. Celery, radishes, spinach and cauliflower are other good choices. Some of the most hydrating fruits are tomatoes, strawberries, watermelon and cantaloupe.

Despite popular folklore, coffee doesn’t cause fluid loss and does provide hydration, but experts say it is still important to limit your amount of caffeine especially toward evening. Drinking too much alcohol does dehydrate the body and one tip to lessen the effects is to consume a beverage high in electrolytes like coconut water. Cold soups are another way to keep your fluid levels high. Oatmeal with berries is a good choice for breakfast as the oats soak up the water they are cooked in and the berries themselves are hydrating.

Hearing Loss and Cognitive Decline

According to the Medical News Today website, studies find hearing loss may contribute to social isolation, lower quality of life, disability, depression, and dementia. This can be especially problematic for people aged 80 years and older, who tend to have more advanced hearing losses.

In 2015, researchers reported on a 25-year study, which focused on a link between hearing loss and dementia. At the start of the study, the 3,670 participants were 65 years or older and followed for over 25 years. Using the Mini-Mental State Examination (MMSE) to measure cognitive decline, researchers say there appeared to be a correlation between self-reported hearing loss and cognitive decline. However, participants who used hearing aids did not show a significant cognitive decline, suggesting the use of hearing aids may offer a protective effect.

Experts say hearing loss may adversely impact a person’s ability to socialize as they miss out on vital verbal and emotional information that may directly impair brain structure and function. Or it could be that the person uses more energy to make sense of what they hear; and reduces the energy they have left for memory, language processing and attention.

It is recommended at age 60, all adults get a baseline hearing test and be rechecked every couple of years, regardless of whether they experience symptoms. John Hopkins Medicine reports one in 3 adults over age 65 has hearing loss, but due to a gradual change in their hearing, they may not be aware of it.

Symptoms of age-related hearing loss, or presbycusis, include the speech of others sounding muffled or slurred; high-pitched sounds, such as "s" or "th" being hard to distinguish; and having difficulty understanding conversation, especially if there is background noise. The person may notice men’s voices are easier to hear than women’s, because it is lower-pitched.

The Importance of Good Nutrition as You Age

The government first published nutritional recommendations in 1941, which became known as the Recommended Dietary Allowance (RDAs). At the time, recommendations were more concerned with ensuring the population “as a whole was adequately fed and free from scurvy, rickets and other wartime diseases of malnutrition.” These early guidelines did not address long-term issues such as heart disease and diabetes.

Since then, we have learned we need to focus on maintaining good nutritional habits well into our senior years to avoid or mitigate our risk for chronic diseases. One example is type 2 diabetes, which occurs when the body’s muscle, fat and liver cells stop responding well to insulin resulting in consistently high blood sugar. Doctors say type 2 diabetes can be caused by too much sugar and refined-carbohydrates in our diet, and to lessen our risk, we should maintain a healthy weight and minimize blood-sugar spikes as well as add more fiber to our diet.

Another thing older adults should be aware of is muscle loss. Along with resistance training, weightlifting, Pilates, and yoga, nutritionists say to maintain strength we need to include protein in our diet. Christine Ritchie, director of research for the division of palliative care and geriatric medicine at Massachusetts General Hospital and a professor of medicine at Harvard Medical School, says older adults’ diet should include more protein than when they were younger. A man in his 50s needs “about 25 to 30 grams at each meal (like a medium chicken breast or burger, a 5-ounce can of tuna mixed with mayo, 2 cups of cooked rice and beans, or a cup of low-fat cottage cheese).”

Government dietary guidelines report that we burn approximately 200 fewer daily calories after age 50, so if you keep eating like you did in your thirties or forties, you will probably gain weight. Also, as bone loss starts sometime in our early thirties and continues into old age, seniors should consume foods with calcium and vitamin D. The American Academy of Orthopaedic Surgeons recommends 1,200 milligrams a day of calcium for women and 1,000 mg for men. Additionally, eat fruits and vegetables in a bright range of colors for heart-protecting antioxidants.

In 2022, the Association added sleep duration to the list as a component of heart health. They say the amount and quality of sleep can influence a person’s eating habits, mood, memory, internal organs and more.

One advantage to getting enough sleep is being more productive. After spending years working long hours to the detriment of a good night's sleep, Ariana Huffington now says, “The irony is that a lot of people forgo sleep in the name of productivity. But in fact our productivity is reduced substantially when we’re sleep deprived.”

Professor of Sleep Medicine at Harvard Medical School, Charles Czeisler, explains being sleep deprived can impair your abilities and is similar to being drunk. Poor sleep habits can cause you to eat more in an effort to feel more energetic, and may increase your risk for high blood pressure, type 2 diabetes, heart disease and stroke.

It is recommended adults get a solid 7 to 9 hours of sleep each day and a sleep routine will help you achieve this goal. Start by going to bed each night around the same time so you can wake up refreshed at the same time each morning. If possible, open your curtains to let in the morning sunshine or turn on the bedroom lights. Research still shows breakfast is the most important meal of the day and should consist of healthy foods.

Another thing to get your day off right is to take a few minutes each morning doing something like practicing mindfulness or yoga. The other Life’s Essential 8 include maintaining a healthy diet, fitness, quitting nicotine, losing weight, and watching your cholesterol, blood pressure and risk for diabetes.

Age-related Muscle Loss

The term sarcopenia refers to age-related muscle loss and is a natural part of growing older. However, it also may occur at a faster pace after prolonged periods of inactivity from injury, illness or the pain of osteoarthritis. Sarcopenia causes muscle atrophy and brings on problems such as poor balance and greater weakness, which in turn, can result in more inactivity and increased risks for falls.

Seniors with muscle atrophy may have difficulty reaching their arms upward, opening jars, typing on a keyboard or even tying shoes. The Harvard Medical School reports findings of a 2015 study on muscle loss in older men taking part in eight weeks of strength training. After stopping for two weeks, the men lost approximately 25 percent of their muscle gains.

Jodi Klein, a physical therapist with Harvard's Brigham and Women's Hospital, suggests a few things you can do if you know you have a surgical procedure ahead of you that could cause muscle atrophy. She says if you are currently active, keep it up; if not, use this time to get started. Klein explains, "The better shape you are in going into surgery, the better shape you will be coming out, and the less chance you have of enduring muscle loss."

Older adults who want to regain muscle strength should talk to their health care provider about the best methods. Experts recommend an exercise routine that builds both upper and lower body strength. This could include weight training with dumbbells and resistance bands in conjunction with walking, swimming, and biking. If it would be helpful, think about turning to an expert to aid you in regaining strength.
