What is Osteopenia?

Osteopenia is a condition that occurs when the body doesn't make new bone as quickly as it reabsorbs old bone. Medical experts say the biggest risk of osteopenia is it can lead to osteoporosis, a more serious, debilitating disease. Both osteopenia and osteoporosis are found in a bone density test, which measures the mass and strength of the bone. If bone density level is low based on the person’s age, they will be diagnosed with osteopenia. A finding of very low bone density is a sign of osteoporosis and causes fragile, easily fractured bones.    

While 54 million Americans have osteoporosis, half of all Americans over the age of 50 have osteopenia. Women post-menopausal are most at risk but one-third of Asian and white men over the age of 50 also have osteopenia. Everyone’s bones get weaker with age but smoking; excessive consumption of alcohol; a diet low in calcium, vitamin D, and certain medications; or medical conditions such as lupus, rheumatoid arthritis, among others puts an individual at a greater risk for osteopenia.   

Typically osteopenia is asymptomatic (until it develops into osteoporosis), and is discovered with a bone density test. The painless test uses a dual-energy X-ray absorptiometry (DEXA) to assess the level of calcium in the bones.  

If you have been diagnosed with osteopenia, it is important to eat foods high in calcium such as leafy green vegetables and dairy; and foods high in vitamin D such as oily or fatty fish, mackerel and fortified cereal. Weight-bearing exercises (at least 30 minutes on most days) are those where your feet touch the ground like running and walking. Swimming and biking are not. For the best health include both weight-bearing and strengthening exercises in your weekly regimen.

Help Prevent Falls with Balance Exercises

The Centers for Disease Control and Prevention’s statistics show that one in four Americans aged 65 years and older falls each year.  Falls are the leading cause of fatal and nonfatal injuries among older adults, and the number of falls will continue to increase as America’s baby boomers grow older.

While these are worrisome facts, experts say there are some simple exercises seniors can do to help prevent falls and maintain their independence. Remember a common injury caused by an elderly person falling is a hip fracture that can seriously impact their life.  

Most balance exercises are easy to learn and can be done anywhere.  The Mayo Clinic explains that while many exercises including walking can help maintain good balance, there are specific exercises targeted at improving a senior’s balance and stability.  They do caution however that if you have severe balance problems or an orthopedic condition, to get your doctor's okay before starting balance exercises.

Examples of balance exercises include: standing on one foot, walking heel to toe, back leg raises, and side leg raises.  To do the standing on one foot exercise, stand behind a sturdy chair; gripping for balance, lift one foot off the floor.  Hold for 10 seconds and then repeat with the opposite leg.  Repeat the exercise several times and increase the repetition as your strength increases.   

Walking heel to toe is done by positioning the heel of one foot in front of the toes of the other foot.  Your heel and toes should be touching.  Now select a spot ahead of you and focus on it to help keep you steady as you walk.  Try to step forward for as many as 20 steps.  Be sure to ask your healthcare provider for other balance exercises.

Common Myths About Arthritis

In an article in Medical News Today experts discuss some of the common myths and facts regarding arthritis, a condition that causes pain and inflammation in joints. Osteoarthritis (OA) and rheumatoid arthritis (RA) are the two main types of arthritis, and a national survey found nearly half of adults 65 years and older have been diagnosed with arthritis. OA, the most common form and leading cause of disability in older adults, is the result of cartilage between the bones in a joint wearing down. 

Many people believe arthritis is only a disease of the elderly but RA, while not as common as OA, typically starts in individuals as young as 20 to 40 years old. People also mistakenly think if a joint hurts, it must be arthritis. Doctors say there are several reasons a person may experience pain in and around joints including old injuries, bursitis, and tendinitis. 

Another myth is you shouldn’t exercise if you have arthritis; however, studies show exercise can actually help with range of motion and strength in the joint (consult with your healthcare provider before starting any new exercise or activity). The American College of Rheumatology says, “Exercise and arthritis can and should coexist. People with arthritis who exercise regularly have less pain, more energy, improved sleep, and better day-to-day function.”

Then there is the debate on whether heat is better than ice for sore joints. The Guy’s and St Thomas’s National Health Service (NHS) Foundation Trust in the UK, explains, “Used in the right way, heat can help reduce pain and stiffness in joints and muscles. Cold application can help reduce joint inflammation and swelling.” Moreover, experts say believing arthritis is unpreventable is only partially true as there are things people can do to mitigate risk or slow down the progression such as maintaining a healthy body weight.

Walking May be the Perfect Exercise

Walking is an exercise we can do while hanging out with friends and/or enjoying the sounds of nature while achieving a good workout. We can do it from right outside our home or by exploring parks and nature areas around us. It doesn’t require special equipment, although experts say you do need a pair of good walking shoes and appropriate clothing including sun protection. *Before beginning any new exercise, please consult with your healthcare provider.

The Harvard Medical School says walking may be “one of the most powerful ‘medicines’ available. It can help lower your risk of heart disease, cancer, and diabetes, lower blood pressure and cholesterol levels, and even keep your memory sharp.” Evidence also shows short walks can be as beneficial as longer ones. In fact just 30 minutes a day can increase cardiovascular fitness, strengthen bones, reduce excess body fat, and boost muscle power and endurance.

Maintaining good posture during a walk is important in avoiding injuries. If you lean forward or hunch, it can cause backaches as well as make breathing more difficult. Experts say extend your spine as if it is being lifted from the crown of your head. Don’t look down at your feet but rather keep your gaze about 10 to 20 feet ahead of you. To get the feel of your spine upright, place your thumbs on your lower rib and fingertips on your hips. Now as you straighten your spine, notice how the distance between your fingers increases. 

If you have sciatic pain, experts say regular walking can be a good exercise because it spurs the release of pain-fighting endorphins and reduces inflammation but also offer some tips to keep you safe. These include shortening your steps, remembering to maintain your posture and slowing your pace. Your healthcare provider should have other tips to help.

Tips to Soothe an Upset Stomach

Every now and then, most of us suffer from an upset stomach. Experts say experiencing a stomach ache once in a while is perfectly normal and can be caused by stress or anxiety, food allergies, eating something that doesn’t agree with you, or a 24-hour bug. To ease symptoms and help you feel better, experts recommend several foods.  

Just as mom told you, chicken-broth is good for an upset stomach as it contains many necessary nutrients. The broth also helps you stay hydrated if you have bouts of diarrhea and vomiting. Plain food like saltine crackers or dry white toast (skip the whole wheat for now) are easy to digest and give your stomach a little something to help it settle down. 

Ginger has been used for centuries to help ease digestive problems including an upset stomach. Drinking ginger tea or eating a couple of ginger chews may help you feel better. Dr. Carolyn Kaloostian, clinical associate professor of family medicine and family medicine specialist at Keck Medicine of USC, says there are several other herbs and spices that can help including licorice, chamomile, spearmint and mint. 

Foods like oatmeal, plain eggs, unseasoned skinless turkey, chicken or fish are all good choices to try to get a little something in your stomach without upsetting it more. Yogurt may be beneficial as it contains probiotics that can help promote healthy digestion but experts advise sticking to low-fat or no-fat varieties.

February is American Heart Month

The CDC says February is American Heart Month, making it the time to focus on cardiovascular disease. Experts say this includes: shining a light on hypertension (high blood pressure), a leading risk factor for heart disease and stroke. The NIH National Heart, Lung and Blood Institute’s National Wear Red Day® is on the first Friday in February. The campaign encourages people to show their support by wearing red and encouraging family, friends and co-workers to do the same. 

Focusing on heart disease is important as it is the leading cause of death in the US for both men and women; and the awareness campaign helps people to realize it is largely preventable. Experts stress that every adult should be aware of their risk and know the healthy choices to make to reduce their chance of heart disease including coronary heart disease, which is the primary cause of heart attacks. 

The NIH explains your risk of heart disease is higher if you:

  • Have high blood pressure

  • Have high blood cholesterol

  • Are overweight or obese

  • Have prediabetes or diabetes

  • Smoke

  • Do not get regular physical activity

  • Have a family history of early heart disease (your father or brother was diagnosed before age 55, or your mother or sister was diagnosed before age 65)

  • Have a history of preeclampsia (a sudden rise in blood pressure and too much protein in the urine during pregnancy)

  • Have unhealthy eating behaviors

  • Are older (age 55 or older for women or age 45 or older for men)

Experts say for each risk you have, the higher your overall risk is for heart disease. While some risks cannot be helped such as your gender, age and family history, it is important to discuss with your healthcare provider what you can do to reduce your risk. Also the NIH says it can be helpful to join #OurHearts in Action, a group designed to encourage and motivate heart-healthy behaviors together.

Use Caution When Using the Internet for Medical Information

Let’s be honest, many of us rely on the Internet for medical advice. While it can supply good answers, doctors also caution you have to be careful what sites you trust. There are many websites providing good medical information but there are also sites with misleading or even dangerous advice.

Dr. Michael Langan, an internal medicine physician at Ohio State University Wexner Medical Center, cautions anyone with a blog can dispense information and advice; and a good way to avoid misinformation is to ask your doctor to recommend the sites they trust. Additionally, the National Institute on Aging recommends using health websites sponsored by Federal Government agencies. They say a good place for reliable, up-to-date information is the National Institutes of Health, but at the same time, remind everyone that the Internet is no substitute for seeing your doctor or other health professional who can give you advice that caters to your specific situation. 

Other things to look for include name recognition such as the Mayo Clinic or the Cleveland Clinic. Moreover, remember sites that end in .gov, identity U.S. government agencies; .edu, educational institutions; and .org, usually designates nonprofit organizations (such as professional groups; scientific, medical, or research societies; advocacy groups). A .com is a commercial site.

The NIH says to look for the date of the information (often can be found at the bottom of the page) and see if the authors and contributors are identified. If the author is listed, what is his/her background and what is their goal in writing the article or the intent of the organization it is written for. Is there any financial motive for the information? Check out the About Us or Contact Us pages as they provide background information about the organization.

Improve Flexibility & Balance by Stretching

Have you noticed you are no longer as flexible as you used to be and the movements of your limbs and joints feel more restricted? If it harder to do things like turning your neck to look behind you when driving or getting up from a low chair, you would probably benefit from incorporating stretches into your routine. Experts say daily stretching can make significant improvement in flexibility, the ability of the muscles and tendons to lengthen and stretch in response to movement, and allow a joint to move through its range of motion.         

In addition being more flexible improves balance and lowers the risk for a fall. Stretching increases the range of movement in the joints, improves posture and eases muscle tension and soreness as well as increases circulation. Along with dedicated stretching exercises, seniors can increase flexibility with yoga, Pilates, swimming, tai chi, dancing and massage. 

A recent study on a 12-month stretch and flex program for older adults reported by the Journal of Gerontology found “participants demonstrated positive changes in areas like physical fitness, self-efficacy, perceived functioning, and well-being. They also experienced a decrease in pain.”  

It is easy to do stretching exercises as they do not require special equipment and can be done anywhere. However before beginning any new exercise routine including stretching, consult with your healthcare provider regarding your fitness level. Experts say you should warm up before stretching such as taking a walk around the block, which in itself helps to stretch muscles and tendons. Also don’t stretch to the point of pain, only to the point of tension; or bounce when stretching. Typically hold the stretch for no more than 30 seconds.

Dealing with High Cholesterol?

If you have been told by your healthcare provider you have high cholesterol, you probably have also been told you will need to watch your diet. The best way to do this is to consume foods that can help lower your LDLs, which are the harmful cholesterol-carrying particles that contribute to artery-clogging atherosclerosis.  

Scientists explain how different foods can help in different ways to achieve this purpose. One type is food containing “soluble fiber, which binds cholesterol and its precursors in the digestive system and drags them out of the body before they get into circulation.” Other foods known to be beneficial for lowering LDLs are polyunsaturated fats, and foods with sterols and stanols, which have been shown to absorb cholesterol.

For 1 to 2 grams of soluble fiber, start your day with a bowl of hot oatmeal or a cold oat-based cereal. Later meals can include barley, whole grains or beans to provide additional soluble fiber. Nutritionists say most Americans only get around half the amount of daily fiber they need (guidelines recommend 20 to 35 grams of fiber a day, and at least 5 to 10 grams coming from soluble fiber). 

Nuts are known to help lower LDLs as well as provide other benefits to the heart. Oils like safflower, sunflower, olive and peanut contain monounsaturated and polyunsaturated fats that help to lower "bad" LDL cholesterol and raise "good" HDL cholesterol. Eating fatty fish once or twice a week such as salmon, tuna, sardines and mackerel, which are loaded with LDL-boosting saturated fats and LDL-lowering omega-3 fats, help reduce triglycerides in the bloodstream.

Help Your Elderly Parents Keep Paperwork in Order

In the event of a medical emergency or illness, to file income taxes, or apply for state or federal benefits, experts say it is helpful and maybe even lifesaving for seniors to have their paperwork in order. Important documents to have available to a family member or trusted friend include property deeds, history of financial income and medical information. For example, Stuart Furman, A Place for Mom legal expert and elder attorney, explains if your parent is eligible to apply for veterans’ benefits or Medicaid, they will need proof of past income and financial resources. 

If you are the designated person, start by helping your parents compile a record of their bank accounts as well as sources of financial income including pensions, 401(k) information, and annuity contracts. In addition help them organize information regarding savings bonds, stock certificates, or brokerage accounts. File these documents along with all property deeds and titles to vehicles in a home safe where they are secure. Files should also include several years of income taxes as well as a list of credit cards and outstanding loans or debts owed to them. 

If your parents have a living will and/or a durable power of attorney, you should have a copy as it is important in the case of a medical emergency to be able to quickly get your hands on such documents. Cataracts are cloudy areas that form in the lens, which is normally transparent, and are the main cause of vision loss in people over 40 years of age. It is estimated more than 40 percent of Americans aged 75 years or older have some degree of lens clouding; and over half of people 80 or older have their vision significantly impaired by cataracts or have had cataract surgery. Doctors say there are things people can do to reduce their risk of developing cataracts with quitting smoking at the top of that list. A healthy diet of fresh fruits, vegetables, whole grains, good quality fats (avocados, olive oil, and omega oils) and lean animal-sourced or plant-sourced proteins additionally reduces the risk of cataracts. 

Maintaining a healthy weight is important as obesity increases your risk of developing type 2 diabetes, which is a risk factor for cataracts. Other ways to reduce your risk include wearing UV blocking sunglasses when in the sunlight and getting at least 7 hours of sleep each night. The risk for cataracts also increases with age, family history, and a history of eye inflammation or injury.

An advanced directive is a legal document giving you a voice involving medical decisions in the event you are incapacitated. Many people shy away from creating such documents as they fail to realize how important they are, and mistakenly worry they are signing away their lives, and/or don’t like thinking about being so ill they are unable to make decisions.    

Advance care planning documents may include a living will, power of attorney and health care proxy. The New York Times published a recent study, which adds to the growing evidence that the time of day we exercise may affect how we benefit from the exercise. The study involved a group of men who were at high risk for Type 2 diabetes; and suggests at least for some, there is a more effective time to exercise each day. For this particular group, researchers say it was more beneficial to exercise in the afternoon rather than mornings. They found afternoon exercise increased the men’s metabolic health and resulted in an improvement in controlling blood sugar and losing belly fat. 

Research has shown in both animals and people that quality of health is influenced by the chronology of our day. The reason is the body’s tissue “contains a kind of molecular clock that chimes, in part, in response to biological messages related to our daily exposure to light, food and sleep.” Our “cellular clocks” calibrate as to when we do normal biological jobs such as divide cells and refuel, and responsible for the dips and crest we go through each day involving our temperature, hormone levels, blood sugar, blood pressure, etc. 

For other individuals, earlier research suggests exercising in the morning may be more conducive to weight loss and burning body fat. Patrick Schrauwen, a professor of nutrition and movement sciences at Maastricht University Medical Center in the Netherlands, says the most effective exercise regimen for each of us will align “with our daily routines” and exercise inclinations. He adds, “I believe doing exercise is better than not doing exercise, irrespective of timing, because exercise is good for us at any time of day — but only if we opt to keep doing it.”

Study Shows the Benefits of Daily Exercise

Even if you have already moved into the “senior” category of life, medical experts say it is not too late to improve your health and mobility, and one of the best ways to do it is to walk. This was the findings based on the Lifestyle Interventions and Independence for Elders (LIFE) trial, which included more than 1,600 men and women between the ages of 70 and 89. Participants consisted of people who were not regularly exercising and fairly frail.  

The participants were divided in half with one group assigned to daily walking and strength and balance exercises. The other group attended educational workshops and were introduced to gentle stretching routines. “After 2½ years, the volunteers in the exercise group were 28% less likely to have become disabled (defined by the inability to walk about 400 yards without help) compared to those in the education group. They were also 18% less likely to have had any episode of physical disability.” LIFE’s researchers say the results could have been more dramatic but for the fact that those in the workshop group learned about the benefits of walking and some started to walk and exercise on their own.  

The advantages of walking include stopping bone mass loss, strengthening leg and abdominal muscles, and improving circulation by helping to lower blood pressure and reducing the risk for heart disease. It helps people lose weight or maintain a healthy weight, sleep better, increase mental sharpness, and improve balance and stability. Doctors say walking can increase your lifespan but if you have not exercised in a while, they caution to slowly increase your pace as well as the distance you walk. If you are unsteady on your feet, use a cane or walker, and everyone should wear properly fitting footwear.

Take 7,000 Steps a Day to Better Health

We have all heard about the value of walking 10,000 steps a day but recent studies show it may not actually be the magic number. One study published by JAMA found walking 7,000 steps or more a day may decrease the risk for mortality and offer better overall health. 

In the cohort study of 2110 adults with a mean follow-up of 10.8 years, participants who walked at least 7000 steps a day, compared with those taking fewer than 7000 steps, to have a 50% to 70% lower risk of mortality. Researchers also noted, “There was no association of step intensity with mortality regardless of adjustment for step volume.”

A 2020 study found a decreased risk for mortality in participants who took 8,000 steps a day than those who only took 4,000 steps a day. In the same year, another study found more steps taken each day were linked to lower rates of diabetes. Researchers say the bottom line is evidence points to the importance of increased physical activity. 

Before you begin walking, talk to your healthcare provider; and if are just beginning to exercise, be sure to start slow. Wear properly-fitted, supportive shoes, and if walking during the day, wear sunscreen, protective clothing and a hat. Experts cite walking as another tried and true exercise for protecting the health of your bones. In a study of nurses, it was found to lower the risk of hip fractures by 41 percent for participants who walked four hours a week as compared to those who walked less.

Could Surgical Hip and Knee Replacements Become Something of the Past?

Researchers say a future of hip or knee “replacements” done without surgery may be on the horizon. By employing a combination of drugs and stem-cell therapies, it may be possible to “repair cartilage before osteoarthritis sets in or an operation is needed.”  

Osteoarthritis, commonly afflicting the elderly, is a degenerative disease attributed to "wear and tear" but can also be caused by earlier sports injuries, obesity and other trauma to the cartilage around the knee, hip or shoulder joints. Experts explain the loss of cartilage, which does not have its own blood supply or nerves, limits its ability to mend itself and is the major cause of joint pain and arthritis affecting over 32.5 million people in the U.S. In 2017 alone, there were 754,000 knee replacements done and 448,000 hip replacements.  

Michael Longaker, a professor of surgery at Stanford University School of Medicine and co-director of its Institute for Stem Cell Biology and Regenerative Medicine, explains the goal of his research is to not wait for osteoarthritis to develop, but to halt the degenerative process so people never need to have a joint replaced. His team’s focus is on programming stem cells to become cartilage cells. Other researchers are working to develop drugs that stop the progression of “wear and tear” on joints. Researchers are also looking at methods to “effectively deliver new cells and compounds” as well as designing materials to help new cells integrate with existing tissue. Scientists admit they still have a ways to go before they will have alternative therapies in place but remain optimistic.
